I have made my Health History form available for you to print. Filling out this form before hand saves about 10-20 minutes of time when you arrive, and makes it so that you can receive you massage sooner!

An accurate Health History form is very important to ensure that it is safe for you to receive massage treatment. There are some medical conditions which require modifications for a safe and effective treatment.

A Health History is also required by law in the province of Ontario and treatment cannot process without it. All information gathered is confidential, except as required by law.
